Provides dedicated community care management for older adults covered by Medicare, offering ongoing support and autonomy.
Responsibilities
- 1099, 30–40 hrs/week in business hours (you set your schedule)
- $30–60 / hour: factors include experience, geography, and client load
- Serve older adults and their families in aging at home safely

- This is not a typical social work or nursing role. If you've felt that traditional healthcare roles have a ceiling on you, and you want to actually bridge the gaps for older adults after they leave the office or facility, Hera is for you. An older adult gets discharged from the hospital and you already know they'll be back in 30 days. A family leaves overwhelmed, clutching a stack of papers with numbers they'll never get through. You did everything you could within the system and it still wasn't enough . We started Hera because we felt the system was letting down the generation that raised us. Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero, you, who actually has the tools, time, and autonomy to help.

- Hold an LMSW, LCSW, or RN (not required but a plus)
- Have experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
- Love fostering connection with older adults and their families
- Are a creative problem solver who wants a job that looks different every day
- Are tech-savvy and excited by quickly evolving environments and leveraging AI to supercharge your work
- Remote - must be located in the United States
